Understanding SIP Response Codes: 200, 401, 404, 486, 500 Explained

When a VoIP call is placed, the Session Initiation Protocol handles the signaling that sets up, modifies, and terminates the connection. Every SIP transaction produces a three-digit response code that tells the sending party what happened to the request. Understanding these codes is essential for anyone who manages, troubleshoots, or optimizes a business phone system.

What Are SIP Response Codes and Why Do They Matter?

SIP response codes are grouped into classes based on their first digit. Codes in the 1xx range are provisional responses indicating that the request is being processed. Codes in the 2xx range indicate success. The 3xx range covers redirection, meaning the request needs to be sent elsewhere. Codes in the 4xx range indicate client errors, such as malformed requests or authentication failures. The 5xx range signals server errors, and 6xx indicates global failures that no server can satisfy.

For administrators and IT staff, these codes provide a precise language for diagnosing call issues. Instead of guessing why a call failed, you can look at the response code and know whether the problem is on the caller side, the callee side, the network, or the server infrastructure. This speeds up troubleshooting and reduces downtime for business communication.

Common SIP Response Codes Explained

SIP 200 OK is the most desirable response. It means the request was successfully received, understood, and accepted. In a typical call flow, you will see 200 OK when an INVITE request is answered, confirming that the call has been established. If you see 200 OK but still experience audio issues, the problem is likely in the media path rather than the signaling.

SIP 401 Unauthorized indicates that the request requires user authentication. This usually means the device or user sending the request did not provide valid credentials, or the credentials were missing entirely. To resolve this, check that usernames, passwords, and realm settings are configured correctly on the endpoint and match the values expected by the server.

SIP 404 Not Found means the server could not locate the user or resource specified in the request URI. This can happen if the dialed extension does not exist, the number was ported away, or there is a routing rule error. Verify that the destination address is correct and that the user is registered on the system.

SIP 486 Busy Here indicates that the callee is currently on another call or has set their status to busy. Unlike a network failure, this is a normal condition. Depending on your setup, the caller might be sent to voicemail, hear a busy tone, or be offered a callback option. If 486 appears unexpectedly, check whether the endpoint failed to clear a previous call state.

SIP 500 Server Internal Error is a catch-all code indicating that the server encountered an unexpected condition that prevented it from fulfilling the request. This can result from software bugs, resource exhaustion, configuration conflicts, or interoperability issues between devices. Review server logs, restart affected services, and check for recent changes to the environment.

How to Troubleshoot SIP Error Codes

Start by enabling detailed logging on your PBX or SIP proxy. Logs will show the full sequence of requests and responses, allowing you to identify exactly where the call flow breaks down. Compare the failing call with a successful one to spot differences in headers, authentication, or routing.

Check network connectivity and firewall settings. Many SIP issues are caused by NAT traversal problems, blocked ports, or incorrect transport protocols. Ensure that devices can reach each other on the required ports and that session border controllers or STUN servers are configured if needed.

Keep firmware and software updated. Manufacturers regularly release patches that fix bugs causing specific response codes. If you encounter persistent 500 errors or unexpected 401 responses, consult the vendor knowledge base or support team for guidance.
